No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. There was no signed Health Reporting Agreement for Kelly, Operator working today. Form was signed during the visit.

Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Two bins of soapy water were observed in two of the three-compartment sink basins with metal lids drying above a towel in the last basin. Operator stated she uses sanitizer solution only at the end of the day. Educated Operator on proper set-up and use of three-compartment sink and proper ppm for chlorine sanitizer.
