Air gap between water supply inlet and flood rim of equipment/sink is less than one inch. Triple sink faucet has been bent down eliminating air gap. Operator raised the faucet to restore the proper air gap.
Toxic substance/chemical improperly stored. In restroom behind ice cream counter, chemicals stored on top of boxes of single service cups.
Handwash sink not accessible for employee use at all times. Employee handwash sink in back room not easily accessible due to cleaning equipment in the way. Hws on front line used to store cleaning bucket. Operator cleared the sinks for use.
No paper towels or mechanical hand drying device provided at handwash sink in unisex bathroom.
