Observed an employee crack raw shell eggs and then handle clean tongs without changing gloves or washing hands. Operator had the employee do both and set the tongs back to be washed.
Employee washed hands with cold water. Observed an employee hands with just cold water. Operator had them rewash their hands with hot water and soap.

Observed butter being held on time without a written procedure for time as a public health control. Educated operator on Time as a Public Health Control. E-mailed copies of forms to culinary director.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed commercially processed salmon bearing a label to remove from packaging before thawing. Educated operator on removing fish before thawing. Operator removed all fish from packaging.
Food not stored at least 6 inches off of the floor. Observed bins of salt, rice, brown sugar, flour and sugar not stored approximately 1 inch off the ground. Operator moved bins to a rack storing them 6 inches off the ground.

Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed split hose without a vacuum breaker.

No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed no proof of employees food handler training. Operator stated she was unable to locate tracing certificates.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed no signed Employee Health Reporting Agreements for cooks. Operator had cooks red and sign health reporting agreements.
Accumulation of lime scale on the inside of the dishmachine. Observed an accumulation of lime build up in dish machine interior.
Dishmachine has no data plate/operating specifications.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee drink on clean dish table in waitstaff area near plates and on make table at the end of cook line. Operator removed both drinks.
Equipment and utensils not properly air-dried - wet nesting. Observed wet nesting on stacked cups in waitstaff area. Educated operator on wet nesting, operator removed cups to be cleaned and sanitized.
