Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. sliced cheddar cheese, sliced Swiss Cheese, raw beef, shredded mozzarella and jack cheese (45F-46F - Cold Holding) in reach-in cooler drawer below grill. Operator stated they had drawer opened during lunch. Operator removed all items from drawer and placed in walk in freezer to chill. Rate per items at end of inspection at 38F-41F
Food-contact surface soiled with food debris, mold-like substance or slime. Portable green cutting board on cook line soiled/ stained. Also, ice chute at door area of in large ice machine soiled. Also, wedger on clean storage shelf soiled.
Handwash sink used for purposes other than handwashing. Observed One of the hand wash sinks in bar area being used for dumping. Operator was not aware that it is on plans as a hand wash sink.
No soap provided at handwash sink. No soap at one hand wash sink in bar area. Operator replenished soap.
Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Some employees food safety training certifications expired in the past two- three months.
Ceiling tile missing. Observed Ceiling tile missing at front service line above water machine and also, ceiling tile missing at prep area above rice cooker area.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed Reach-in cooler drawer below grill unable to maintain temperature of 41F and below. Ambient temperature 54F-58F. Observed that the drawer couldn't fully closed and the gasket on drawer torn. Operator removed TCS foods from drawer and placed in walk in freezer to chill.
Equipment and utensils not properly air-dried - wet nesting. Clean stacked pans on storage shelf in warewashing area unable to properly air dry. Operator had employee spread out pans to properly dry.
