Carbon dioxide/helium tanks not adequately secured. Behind front counter on floor next to soda machine, 2 co2 tanks not properly secured. Operator secured tanks with chain during inspection.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Vents in dining area above pizza boxes and vents in kitchen have dust buildup.
Employee with no hair restraint while engaging in food preparation. Operator does not have a hair net, but put on a hat during inspection.

Food stored on floor. Behind front counter, 2 cases of water bottles stored on the floor. Under shelf next to pizza oven, case of pineapple and case of olives stored on the floor. Operator moved everything off the floor.
