Live, small flying insects found Observed approximately 5 small flying insects flying around and above breads and bread slicer at front prep counter in kitchen.
Whole meat roast hot held at less than 130 degrees Fahrenheit. Observed whole roast corn beef roast (123F-hot holding) inside hot box unit at front prep counter. Per operator, items placed inside hot box unit and left out of temperature for approximately 1 hour. Operator closed hot box door all the way and turned heat element to bring temperature up.
Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Observed at front prep area reach in cooler- smoked salmon and white fish platter inside sealed plastic container wrapped in plastic at 55F - Cooling at 10:00 since 9:00-55F at 10:30) At current rate of cooling, food with not reach 41F within 4 hours. Operator removed plastic wrap and lid and placed platter inside walk in cooler to quick chill.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed clean food container at prep table soiled with slime in back kitchen.
Handwash sink used for purposes other than handwashing. Observed a pair of soiled plastic gloves inside hand wash sink in kitchen,operator removed.

Observed employee handle dirty food processor at triple sink, then proceed to put on new pair of gloves and handle bag of peppers and begin food preparation with out washing hands first. Operator educated employee to wash hands correctly.
