Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Soiled with dust, with condensation leaking near clean dish storage in dishwashing area.
Equipment and utensils not properly air-dried - wet nesting. By prep line left side kitchen. Operator separated to air dry.

In-use utensil stored in standing water less than 135 degrees Fahrenheit. In upstairs bar, drink making utensils in standing water 58F.
Unclean building components, attachments or fixtures. Dust on electrical cords and storage over prep line tables over utensil hanging storage - also heavy condensation on vents in same area, not currently dripping onto clean equipment or utensils, per operator wiped down routinely to prevent contamination.

Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed bartender on second floor garnishing drink with olives, no gloves. Spoke with operator regarding proper glove use.
No soap provided at handwash sink. First floor bar handwash sink, 2 handwash sinks men's restroom first floor, front part. Operator refilled all dispensers.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Large ice machine service station first and second floor.
