Food-contact surface soiled with food debris, mold-like substance or slime. Observed food prep tables heavily soiled with food debris. Observed single door flip top cooler with food debris buildups.
Handwash sink not accessible for employee use at all times. Observed handsink blocked with pots and pans. Owner moved.
Manager or person in charge lacking proof of food manager certification. Observed no manager certification.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand sink. Owner replaced.

Cutting board has cut marks and is no longer cleanable. Observed grooved cutting board on flip top cooler unit.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed food prep employee with wristwatch and bracelets on while preparing food. Employee removed.
Food stored on floor. Observed bottles of cooking oil stored on the floor. Owner moved to shelf.
Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle. Observed no license number on unit.
No handwashing sign provided at a hand sink used by food employees. Observed no handwashing sign at sink.
Single-service articles improperly stored. Observed to go pizza boxes stored on the floor.
Stored food not covered. Observed open can of pineapple not covered. Owner covered can with foil.
