Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. Observed women's and men's bathroom doors at dinning area not completely self-closing.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed RIC unit below flat top with a temperature of 99F. Operator has other refrigeration units available.
Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Observed dish washer, that is not in use, with a chlorine concentration of 0 ppm.
