Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. "Y" connector at mop sink is missing vacuum breaker on the non-chemical side of the "Y" connector. Manager put vacuum breaker on non-chemical side.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Mold like substance on the underside of the non food contact surface of the nozzles on the margarita dispensing machine at bar.Employee cleaned.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Inside warmer at left side of cookline: beans (112F - Hot Holding). Manager stated he cooked the beans approximately one hour prior and proceeded to reheat them to 180F. beans (112F 3:20pm, 180F 3:30pm - Reheating)
Dead roaches on premises. One dead roach sitting on lip of reach-in freezer in prep area. Manager cleaned and sanitized the area during this inspection.
Equipment and utensils not properly air-dried - wet nesting. Wet nested drink cups on counter beside fountain machine in lobby. Manager restacked the cups to allow them to dry during this inspection.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. 1. Bottom interior of reach-in freezer at cookline soiled with food debris. 2. Stickers stuck to top of table near warewash area. Manager removed stickers and sanitized the table during this inspection.
Working containers of food removed from original container not identified by common name. Containers of salt and of sugar missing labels on shelf in prep area. Manager added the proper labels during this inspection.

Open dumpster lid. Sliding doors on two dumpsters left open. Manager closed sliding doors during this inspection.
