Current Hotel and Restaurant license not displayed. License currently displayed expired June 1 2026. Manager had owner send new license and displayed.
Floor, wall and/or ceiling soiled/has accumulation of debris. Floor in walk-in cooler soiled with food/rusty. Also, ceiling tiles around hood system stained.

Pesticide-emitting strip present in food prep area/storage. Blue light sticky plug in trap under front counter next to single service wrapped straws. Operator removed single service items from area.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In steam table by drive through, marinara (112F - Hot Holding); meatball (100F - Hot Holding). Items have been in steam table approximately 1.5 hours. Operator added them to time as a public health control procedures.
Toxic substance/chemical improperly stored. On bottom shelf of boxing station, spray bottle of disinfectant stored next to clean pizza trays. Operator removed spray bottle from shelf.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice machine soiled with white residue build up.
Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. Blue light sticky plug in trap under front counter has approximately 20 dead flying insects in it. Operator stated he will change today.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Under prep table in back kitchen, employee jacket on top of multiple cans of food products. Employee removed belongings and placed them in office. Also, employee cellphone on wheeled cart near prep table with clean equipment on it. Employee removed phone from cart.
Equipment and utensils not properly air-dried - wet nesting. Multiple clear pans on dish rack by dish stacked before properly air drying.
Floor tiles missing and/or in disrepair. Multiple floor tiles under pizza oven broken/missing.
Floor, walls and/or ceiling soiled/has accumulation of debris. Floor under ice machine soiled with build up. Also, ceiling tiles near hood system heavily stained. Also, multiple ceiling vents soiled with mold-like substance/dust. Also, wall under dough station table soiled.

A return visit. The problems from the previous inspection had been dealt with.

Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Approximately 13 live flying insects observed in back kitchen area.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In reach in cooler by drive through area, ranch dressing 53F. Ranch dressing placed into cooler approximately 1.5 hours prior. Ranch dressing moved to walk in cooler to cool.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In metal bucket on prep counter by drive through, portioned marinara sauce 126F. Sauce placed out approximately 15 minutes prior. Sauce was added to time as public health control procedures.
Toxic substance/chemical improperly stored. Spray bottle containing yellow chemical cleaner stored on utility cart with salt and oil. Bottle moved and stored correctly.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Vacuum breaker missing on outside faucet in outside seating area. Vacuum breaker missing at the end of both sides of the splitter for outside faucet in dirt area behind establishment. Manager states he will buy today.
Employee personal food not properly identified and segregated from food to be served to the public. In small reach in cooler near front counter, employee food on bottom shelf not identified. Manager identified food.
Establishment did not report seating change that affects the license fee, Clean Indoor Air Act, sewage system approval or other related requirements. Establishment is licensed for 57 seats but has 80 including outside seating. Inspector provided Seating Change Evaluation Form.
Food stored on floor. Bags of cornmeal and sugar stored on floor near walk in cooler. Items moved and stored correctly.
