Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In the top portion of the flip top reach in cooler on the cook line; ham (49F - Cold Holding) stored above the containers of the cooler. Operator relocated the ham to three door reach in cooler.
Food-contact surface soiled with food debris, mold-like substance or slime. Can opener on the prep line with food debris on the blade.
No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Time as public health control written plan from previous ownership. Provided new paperwork to be completed.

Buildup of food debris/soil residue on equipment door handles. Reach in chest freezer in storage shed with mold like substance observed on the handles. Employee cleaned the door handles.

Dead roaches on premises. Observed two dead roaches in the dry storage prep room area on the floor. Employee swept and discarded the dead roaches.
Equipment and utensils not properly air-dried - wet nesting. On the shelf over the three compartment sink, clean plastic containers stacked while wet.
Floor soiled/has accumulation of debris. Build up of debris under tables in dry storage prep room area.
Ice buildup in reach-in freezer and/or walk-in freezer. Chest freezers in sheds with ice build up around the seal.
In-use tongs stored on equipment door handle between uses. Tongs stored on cookline oven door handles. Operator relocated the tongs.

Presence of insects, rodents, or other pests. Observed one lizard on the shelf of the dry storage area, operator caught and removed the lizard.
Shell eggs in use or stored with cracks or broken shells. See stop sale. In the standup reach in cooler, one cracked shell egg in cardboard flat. Operator removed and discarded.
Toxic substance/chemical improperly stored. Chemical spray bottle stored on shelf facing clean utensils over the three compartment sink. Operator relocated the chemical spray bottles.
Food-contact surface soiled with food debris, mold-like substance or slime. Soda dispenser in the waitstaff area has residue build up in the soda nozzle. Bulk ice bin has mold like substance in the corners of the deflector plate. Operator had employee begin to clean both. Microwaves on the main cook line has build up of food debris.
No paper towels or mechanical hand drying device provided at handwash sink. No paper towels at hand wash sink in the mens restroom. Operator replaced the paper towels.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee phone, keys stored on prep table with raw shell eggs stored on time. Operator relocated the personal items.
Equipment in poor repair. Standup reach in cooler closest to the rear exterior door has torn cooler door gaskets.
Floors, walls and ceiling soiled/has accumulation of debris. Accumulation of debris on the floor of the dry storage room under equipment. Build up of grease on wall behind hand wash sink on the main cook line.
Food stored on floor. Oil stored on the floor of the exterior dry storage shed. Root beer syrup box on the floor in front of the soda dispenser. Operator relocated the items from the floor.

Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. Observed employee crack shell eggs with gloved hands, then handle clean plate to plate fries. Spoke to employee during inspection.

Stop Sale issued due to food not being in a wholesome, sound condition. Flying insect landed on sausage link. Person in charge discarded.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In fliptop hotdogs at 54F, items stored above fill line, person in charge moved to drawers below. Also, in white reach in cooler creamer at 52F ambient temperature of unit 55F, person in charge states had unit open to stock, moved to another unit holding temperature. Also, items in freezer portion of white cooler slacking.
Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking. Missing time mark on pancake batter, person in charge added time to board.
