Cold holding equipment not maintained in good repair, two reach-in cooler out of order at this time. Smoothie make station is keeping foods cold by use of ice, tall standing reach-in cooler is not being used until maintenance is performed.
Cutting boards have cut marks and are no longer cleanable, operator stated they have the equipment to sand them down.

In-use ice scoop stored in soiled pan between uses, operator removed and sanitized both immediately.
Insect control device hanging is not designed to retain insect within the device, operator stated that it doesn't work, and removed it.
