Observed operator remove and put into dish; Observed white cutting boards stained throughout on line with food stains. Observed operator get a new one and put into dish to wash, and threw away old one. Observed can opener with food debris on blade. Observed operator put opener into dish to be washed.
Equipment and utensils not properly air-dried - wet nesting. Observed wet nesting with blenders. Observed operator separating blenders.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gaskets soiled gasket on reach in cooler at prep table with some food debris.

Working containers of food removed from original container not identified by common name. Observed a powder substance in a food grade container, out of original packaging,without an identifying label on it. Observed operator putting a label on it.
