Food-contact surface soiled with food debris, mold-like substance or slime. Observed staining on green cutting boards in clean dish area. Person in charge states has new one on order. Also, ice scoop storage container with debris buildup.
Required employee training provided by a third-party employee training program and operator unable to provide original certificate for employees trained. Observed several employees food handler training with copies of original certificate.
A return visit. The problems from the previous inspection had been dealt with.

Some items stored above fill line, lid open frequently per person in charge. Items have been out of temperature for less than 4 hours per person in charge. Person in charge removed time/temperature control for safety and moved to walk in cooler to rapidly chill.

Food-contact surface soiled with food debris, mold-like substance or slime. Portable cutting board at counter next to sandwich making station stained/ soiled. Also, the underside of water machine across from front counter slightly soiled.
Clean equipment/dishware/utensils stored below soap or paper towel dispenser exposing the items to drips from people's hands. A clean mixer container stored below soap dispenser near hand wash sink near drive thru window. Employee removed blender container to properly store.
