Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. At make line flip top reach-in cooler, cheese (51F - Cold Holding); cut tomato (46F, 50F - Cold Holding); cut lettuce (47F - Cold Holding). Operator stated items placed in cooler 30 minutes prior. Ambient thermometer shows 50f. All items moved to sheet trays and placed in walk-in freezer. All items 33f-35f after 30 minutes.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In hot box near convection oven, mac and cheese (129F - Hot Holding). Operator stated mac and cheese placed in warmer 30 minutes prior to. Operator stated door opened constantly. Mac and cheese moved to oven to reheat, 206f after 45 minutes.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Mop sink in storage room in dining area, backflow preventer missing on non chemical dispenser side of splitter.
Handwash sink not accessible for employee use at all times. In side room walk up window, handwash sink blocked by sign and merchandise stand. Operator moved items.
