Single-use gloves not changed as needed after changing tasks or when damaged or soiled. In the middle of prepping a burger dish, observed operator scratch is arm with a gloved hand. Operator proceeded to dress the burger with ready to eat ingredients before washing changing gloves.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Flip top (right); tomato based sauce (40-50F - Cold Holding). As per operator food in unit less than 4hrs. Not portioned or prepared today. Observed flip top kept open. Food warm on top portion, cold on bottom portion. Operator closed flip top for quick chill.
