Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. In reach in freezer, open box of raw shrimp stored on top of bag of sweet potato fries. Operator removed raw shrimp from freezer and stored on bottom shelf of different freezer.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. At front counter, Tapioca pearls (94F - Hot Holding). Operator stated pearls were cooked 1 hour prior and turned warmer up.
Toxic substance/chemical improperly stored. On storage shelves to left when entering kitchen, bottle of first aid brown liquid and bottle of hydrogen peroxide stored on shelf next to mini macaroons and above single service items. Operator removed chemicals from shelf.
Food-contact surface soiled with food debris, mold-like substance or slime. Nozzle of green tea container at front counter soiled with white residue.
Handwash sink not accessible for employee use at all times. Handwash sink at front counter observed with plastic gloves and to go bowls in sink. Employee removed all items.
Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Handwash sink in kitchen broken. No water available at sink.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior deflector plate of ice machine soiled with black mold-like substance.

In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. On shelf to left when entering kitchen, handles of scoops in mango, jelly and egg powder. Operator removed handles from powder.
Insect control device installed over food preparation area. Bug light on prep/cook line table in back kitchen.

In reach in freezer at end of cook line, plastic bags of raw chicken stored over open box of raw whole fish. Manager moved raw chicken to be under fish and beef products.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice machine at bottom of deflector plate has few black mold-like spots.
Bowl or other container with no handle used to dispense food. On bottom shelf of cook line prep table, multiple containers of products have to go bowls used as scoopers in them. Manager removed all to go bowls and replaced with measuring cups with handles.
Employee drinking from an unenclosed beverage container in a food preparation/clean dishware area and the container does not have a dispensing mechanism to prevent hand contact with lip contact area of drink container. Employee open drinking cup on front counter in front of counter top reach in cooler and tea prep station.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Missing at the end of both splitters for mop sink faucet where no hose is attached. Splitter removed (faucet has built in breaker).
