Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Observed hand washing sink near dish washing area with slow drain.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee handled raw chicken, changed gloves then handled Togo boxes without washing their hands. Employee discarded the Togo boxes, washed thier hands and changed gloves.
Server handled soiled dishes or utensils and then picked up plated food, served food, or prepared a beverage without washing hands. Server handled dirty dishes then a bottle of unopened beer without washing their hands. Server voluntarily discarded the bottle and washed their hands.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Fried Rice (63F - Cold Holding) stored at room temperature on the cook line. Manager stated the rice had been stored at room temperature for approximately 45 minutes. Manager placed the rice in the walk in cooler.
Toxic substance/chemical improperly stored. Individually wrapped chop sticks stored below gallon of cleaning fluid in the back kitchen area. Owner removed the chopsticks.
Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Raw sushi fish not identified as a raw items on the menu.
Accumulation of black/green mold-like substance in the interior of the ice machine.
Time/temperature control for safety food packaged using a reduced oxygen method without a HACCP plan not removed from its package within 48 hours after packaging. Observed reduced oxygen packaged raw beef stored in the reach in freezer. Owner stated the beef was packaged on site using a reduced oxygen packaging method. Owner removed the beef from the packaging and placed it in the reach in freezer. Explained the HACCP procedure to the owner, the owner stated they would be using zip lock bags for now on.
