Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. Observed the shared employee restroom not self closing.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed the back toaster oven with black build up.
Outer openings not protected with self-closing doors. Observed the back door leading outside from the prep area not self closing.

Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw bacon stored over ready to eat milk in the front kitchen reach in cooler. Operator moved the bacon.
Shell eggs not stored at an ambient air temperature of 45 degrees Fahrenheit or less. Observed on the back cook table and back reach in cooler respectively: Ambient shell eggs (68F - Cold Holding); Ambient shell eggs (53F - Cold Holding). Operator stated that the cook table eggs had been out for approximately 1/2 hr and placed them in a reach in cooler. Operator stated that the reach in cooler eggs had been there for approximately 2hrs and placed them in a different reach in cooler.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed in the front make table and and back reach in cooler respectively: Sliced tomatoes (54F - Cold Holding); Uncooked Bacon (54F - Cold Holding). Operator stated that the tomatoes had been out for approximately 1hr. Operator placed the tomatoes in a reach in cooler. Operator stated that the bacon had been in the cooler for approximately 1hr. Operator placed bacon in a different reach in cooler.
Employee washed hands in a sink other than an approved handwash sink. Observed an employee wash their hands with the kitchen triple sink. Instructed operator on proper procedure.
Handwash sink not accessible for employee use at all times. Observed the kitchen hand wash sink with dirty dishes and a container of bacon blocking it. Operator removed the items.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at the front server hand wash sink. Operator placed more towels.
No soap provided at handwash sink. Observed no soap at the kitchen hand wash sink. Operator placed soap.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed build up in portable ice machines on bottom shelf in hostess area.
Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Observed throughout kitchen and prep areas.
