No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Operator does not have test strips for quaternary sanitizer used in triple sink. Provided operator with test strips
Dead roaches on premises. 1 dead roach underneath refrigerator in kitchen. Leg and wing of dead roach found in area with cleaning supplies. Operator discarded, cleaned and sanitized areas.
Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Chlorine less than 50ppm. Operator ran cycle 3 times and dishmachine is now at 50ppm.
