Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Triple Sink (Quaternary 0ppm). Manager checked sanitizer on wall and there was hole in line so he manual poured sanitizer into three compartment sink and rechecked at 150ppm.
Ready-to-eat, time/temperature control for safety food not consumed/sold within 7 days after opening/preparation. See stop sale. In stand up reach in cooler near salad cooler, three containers of baked cavatappi expired 8/25/26.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In walk-in cooler on speed rack, three containers of cooked pepper onion mix (49F-50F - Cold Holding); two containers of caramelized onions (50F-51F - Cold Holding); spicy sausage (51F - Cold Holding); two containers of marinated chicken (51F - Cold Holding). Operator stated prep employee had speed rack out for no longer than an hour and put back into walk-in cooler.
Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. In stand up reach in cooler near salad cooler, container of cooked onions made day prior with no date mark. Operator labeled container.

Floor, wall and/or ceiling soiled/has accumulation of debris. Floor in dish area under dish machine soiled with food debris. Also, floors and walls in dough walk-in cooler soiled with black mold-like substance. Also, floor in bar area under ice bin/handwash sink/dish machine soiled with residue build up.
Garbage can located outside has no lid or lid open/broken. Outside dumpster on property both lids broken off.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets on reach in coolers on cook line soiled/torn. Also, portable fans used on floor soiled with dust and food build up. Exterior of ice chute on soda machine in lobby soiled with mold-like substance.
Water leaking from pipe and/or faucet/handle. Spray nozzle in dish area leaking water while handles are turned off.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. In back prep area, Sanitizer Bucket (Quaternary 0ppm). Manager checked sanitizer on wall and the line as hole in it so he poured some sanitizer into bucket and rechecked at 200ppm.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed wings (54F/57F - Cold Holding); macaroni and cheese (53F - Cold Holding)on cart by pizza ovens. Operator states both products placed out of the cooler one hour prior. Both products moved to the freezer and approximately 20 minutes later, wings at 38F and macaroni and cheese at 42F.
