Operator is doing a special process, conducting reduced oxygen packaging, without a HACCP plan approved by the Division of Hotels and Restaurants. Observed andouille sausage in vacuum sealed bags, marked with previous day's date, less than 48 hours, but not marked with the time. Employee removed sausage from bags. Discussed 48 hour plan and HACCP plans with manager.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin.
Flooring in disrepair. Soft spot in walk-in cooler floor in front of door.
Food-contact surface not smooth and easily cleanable. Cut marks and a large worn spot in the center of the prime rib cutting board. Manager placed cutting board in trash area during inspection.

Observed raw chicken stored in same container with shrimp, scallops and grouper. Manager moved chicken to bottom pan with other chicken and moved other products to clean container.
Ready-to-eat, time/temperature control for safety food marked with a date that exceeds 7 days after opening/preparation. Observed cut cooked onions dated 9/22 and risotto dated 9/26 in reach in cooler in kitchen prep area. Kitchen manager threw items away.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed sausage patties at 122F for 2 hours. Kitchen manager put patties back in oven to reheat.
