Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Ceiling soiled with dust near walk-in cooler.
Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Sanitizer solution and rinse aid buckets on the wrong line. Operator rearranged and tested a few times. Tested 100ppm.
A return visit. The problems from the previous inspection had been dealt with.

No paper towels or mechanical hand drying device provided at handwash sink. At employee bathroom. Operator added paper towels.
