Observe ground beef in double pan in deli cooler in individual bags. Top of ground beef was 48 degrees bottom of ground beef was 42 degrees establishment took ground beef put on sheet pan and put brought it into the walk-in freezer recheck temperature at 37 degrees. Overfilled Spoke with establishment about overfilling.

Handwash sink used for purposes other than handwashing. Prep cook used dropped a lime on floor took lime to hand wash sink near walk in cooler to rinse lime off. Educated. Took lime to three compartment sink to rinse off.
Building components, attachments or fixtures in poor repair. Observed outside above back door heavy water leaking from possible AC unit. No rain in area but looked like rain off roof heavy mold like substance needed.

Operator is not properly tracking/marking the number of days opened commercially processed, ready-to-eat, time/temperature control for safety food was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. Meats, vegetables repackage and not time tracked/Date marked.
