Observed employee handle dirty dishes, placed dishes in dishmachine, handled cell phone, then put together plated food for a take out order without washing hands first. Manager coached employee on proper handwashing.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed cut tomato (57F - Cold Holding); hash brown (50F - Cold Holding); ham (53F/51F - Cold Holding); cut lettuce (52F - Cold Holding)in flip top reach in cooler to left of the grill. Per manager, unit was turned off to defrost and turned back on approximately 30 minutes prior to temperatures being taken.
Handwash sink used for purposes other than handwashing. Scrub pads stored in kitchen handwash sink. Employee removed scrub pads.
No currently certified food service manager on duty with four or more employees engaged in food preparation/handling. Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee handle raw shell eggs, remove gloves and place on new gloves without washing hands first.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Wiping cloth solution at far right of line tested at 200+ppm chlorine. Employee remade solution to 100ppm chlorine.
Equipment and utensils not properly air-dried - wet nesting. Observed employee take wet pans from dish area, stacked the wet pans and placed on shelving.
Single-service articles not stored inverted or protected from contamination. Take out cups in holders at drink stations have top cover removed. Employee replaced top cover lid on all take out cups in holders.

Wet wiping cloth not stored in sanitizing solution between uses. Three wet wiping cloths at three different sanitizer solution buckets are stored at back edge of container and not in the water. Manager put all wiping cloths back into each solution.

Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. In walk in freezer, open case of comminuted beef steaks stored above pork chops.
Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking. At approximately 8:55, inspector observed shell eggs above grill, being held on time controls, with no time marking. Per manager, shell eggs were placed on time control at 8:00 and marked time chart appropriately.
No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Per manager, approximately 18 employees employed over 60 days. Only two proof of training certificates located which both expired on 8/9/2024 for Kaylen A and on 8/3/2024 for Anthony T. No other proof of food handler training available.

Three employees(Heather, Desmond and Jayton) hired less than 60 days ago, per manager are missing proof of food handler reporting responsibility.
Garbage not placed in a receptacle for storage until pick up to make the garbage inaccessible to insects and rodents. Observed loose and bagged garbage around dumpster inside enclosure.

Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. Observed cook crack raw shell eggs then handled clean spatula and clean plate to remove and serve cooked bacon without changing gloves and hand wash. Observed employee scrubbing dirty dishes, placed on rack and put in dishmachine. Employee then removed clean stainless pan inserts to put away without washing hands. Inspector spoke with manager about proper handwashing procedures.
Toxic substance/chemical improperly stored. Bucket of chemical cleaner on top of boxes of hash browns in dry storage by walk in cooler. Manager moved chemical to proper storage.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice machine on sides and on deflector plate has black mold-like substance.
No handwashing sign provided at a hand sink used by food employees. No employee hand washing sign at hand wash sink across from the office. Provided DBPR HR 5030-111.

Cook then handled utensils, removed cooked bacon from the grill and continued handling food without washing hands and changing gloves.

Per manager, hash browns placed in cooler approximately 20 minutes prior. Hash browns placed in walk in freezer. Temperature taken approximately 30 minutes later at 41F. Sliced cheese in reach in cooler next to the grill at 51F. Observed cheese stacked above the chill line. Per employee, cheese placed in cooler 15 minutes prior. Cheese moved to walk in freezer. Temperature taken approximately 20 minutes later at 34F.
Wiping cloth solution exceeding the maximum concentration allowed stored in a location that could result in the cross contamination of food, equipment, utensils, linens, single-service, or single-use articles. Sanitizer bucket at door to back area tested at 200+ppm chlorine. Employee remade solution to 50ppm chlorine.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Cook wearing a bracelet.
