Wiping cloth chlorine sanitizing solution not at proper minimum strength. 200 ppm. The manager discarded the chlorine solution and made a container of new solution at 100:ppm.

Bowl or other container with no handle used to dispense food. Employee used a cup to scoop the ice from the ice bin at the beverage station.
In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Scoop inside the hash browns in the top section of the reach in cooler on the cook line. The handle was removed.
