Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Observed no time stamp on basket of raw eggs over flat top. Operator replaced eggs with fresh basket and time stamped.
Equipment and utensils not properly air-dried - wet nesting. Observed clean metal pans stacked and wet nesting on shelf above Three compartment sink.

Observed wash cloth sanitizer solution at 200ppm chlorine. Operator refilled bucket, now reads 100ppm chlorine.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed inside of ice machine with black mold like substance inside bin.
Equipment and utensils not properly air-dried - wet nesting. Observed metal pans wet nesting on shelf over three compartment sink.
No Heimlich maneuver/choking sign posted. Provided Operator with DBPR Form 5030-105 via email. Operator located a laminated one and hung up during inspection.
Single-service articles improperly stored. Observed to go containers stored on floor of dry storage. Operator moved to shelf.

No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand washing sink next to three compartment sink. Operator refilled during inspection.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee wearing bracelet while preparing food.
Light shield damaged/in disrepair. Observed cracked light shield in hallway outside restrooms.
Opened employee beverage container in a cold holding unit with food to be served to customers. Observed employee beverage in tall reach in cooler across from manager office sitting on package of sliced cheese. Operator removed drink.
Single-service articles not stored inverted or protected from contamination. Observed to go coffee cups in stainless dispenser with no cover protecting cup openings. Operator covered.
