Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Upon inspection, observed 10 ppm reading for dishmachine. Employee checked sanitizer container and primed sanitizer for machine and 50 ppm reading was observed.

Dead roaches on premises. Upon inspection observed dead roaches in the following locations: 5 inside ceiling light covering in dishwashing area, 1 inside light ceiling covering in kitchen area, 2 inside ceiling light covering in cook line area, 2 on floor in dry storage area.
Food stored on floor. Upon inspection, observed cans of tomato puree and ketchup store on floor in dry storage area.

Stop Sale issued due to food not being in a wholesome, sound condition. Upon inspection, observed 2 units of thawed tuna sealed in vacuum packages in reach-in cooler near front counter of establishment. Operator discarded packages of fish during inspection.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Upon inspection, observed egg rolls 45f, springs rolls 45f in reach-in cooler. Operator stated items have been in cooler for less than 4 hours. Operator moved both items to colder holding unit for cooling during inspection.
Toxic substance/chemical improperly stored. Upon inspection, observed bottle of bleach stored on top of bucket of soy sauce in kitchen area. Operator moved container of bleach to another location in kitchen during inspection.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Upon inspection, observed 2 units of thawed tuna sealed in vacuum packages in reach-in cooler near front counter of establishment.
Current Hotel and Restaurant license not displayed. Upon inspection, current license not displayed at establishment. Operator was able to show proof of current license on laptop during inspection.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Upon inspection, observed build-up of grease on inside of kitchen hoods at establishment.
Stored food not covered. Upon inspection, observed storage bin on storage rack of Rangoon in walk-in freezer uncovered.

Operator was unable to provide proof of training for employees during inspection. All employees over 60 days.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Tempura shrimp 74f for 1.5 hours.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. No chlorine test strips available. Inspector used own strips to verify chlorine level. Inspector left some strips with operator.
