Handwash sink not accessible for employee use at all times. Handwash sink blocked by trash can and large containers of oil. Manager unblocked the sink during this inspection.
Handwash sink used for purposes other than handwashing. Sanitizer bucket stored down inside handwash sink. Manager removed the bucket from the sink during this inspection.

Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Inside reach-in cooler: Four packages of fish thawed inside reduced oxygen packaging. The fish is completely thawed. Cutting board has cut marks and is no longer cleanable. Cutting board at refrigerated make-table has deep cut marks and is no longer smooth and cleanable.
Food stored on floor. Boxes containing tortillas and seasoning stored directly on floor. Manager removed both items from the floor during this inspection.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gasket on door of reach-in freezer soiled with mold-like substance.

Ziplock bag containing raw beef stored on shelf directly above bag containing onion rings inside reach-in freezer in back of trailer. Manager moved raw food to the bottom shelf during this inspection.

Manager on-duty stated that Tony is still in-charge of the kitchen and MFDV. Tony was the manager in-charge during the last inspection.
