Raw animal food stored over/not properly separated from ready-to-eat food. In Walk in cooler, observed raw fish being stored over cooked potatoes. Operator rearranged products.
Menu contains an item made with a raw/undercooked animal food as an ingredient and description of item does not inform consumer of raw/undercooked animal food ingredient. Menus offer crudo,tartare,bernaise and carpaccio with no secondary symbol like an asterisk so consumer advisory applies.
No paper towels or mechanical hand drying device provided at handwash sink. At Hand washing sink in server station observed no paper towels.

Bowl or other container with no handle used to dispense food. Observed Quart container with no handle in panko bread crumbs. Operator removed container.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Utensils in 104f water at front line, operator discarded water.

The operator stated the items had been held for approximately 3 hours. The operator voluntarily discarded the potentially hazardous foods. Observed liquid egg (52F - Cold Holding); butter (54F - Cold Holding) on the cook line. The operator was emailed a time as public health control form.
