Employee rubbed hands together for less than 10-15 seconds while washing hands. Employee at cookline handled raw hamburger meat, then washed hands for approximately five seconds, then put on new gloves. Manager had the employee wash hands again for 20 seconds and put in new gloves.
Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee at cookline wiped bare hands on pants, then proceeded to handle the fryer basket without first washing hands.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. One employee working at cookline and handling food with bracelet and watch on wrist.
Food stored on floor. Two cases of lemonade stored directly on the floor inside the walk-in freezer. Manager placed both cases on a shelf during this inspection.

Employee jacket and gloves stored on top of fountain soda syrup boxes on shelf in kitchen near ice machine. Manager removed items from top of soda syrup boxes during this inspection.
Ice scoop handle in contact with ice. Handle of ice scoop stored down inside ice inside ice machine in back of kitchen. Manager placed ice scoop in proper position during this inspection.
Missing drain plug at dumpster. Drain plug missing from cardboard dumpster beside dumpster enclosure.
Unclean building components, attachments or fixtures. Green wire shelf above prep sink in kitchen soiled with dust.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Sanitizer bucket quaternary 0ppm. Manager dumped solution and refilled the sanitizer from the three-compartment sink, then 300ppm.

Employee has not received adequate training related to their assigned duties as evidenced by the inability to answer basic food safety questions. Dishwasher provided quaternary test papers to test chlorine dish machine. Inspector educated employee during this inspection.
Fly sticky tape hanging over food/food preparation area/food-contact equipment. Fly sticky tape hanging from ceiling above prep area beside hood at cookline. Manager had employee removed sticky tape from ceiling during this inspection.
Food stored on floor. One box of fountain soda stored on floor in kitchen near ice machine.
