Food stored in a location that is exposed to splash/dust. At front counter, cookies display case stored in location of water dripping from ceiling. Operator moved display case to other location away from drip line.
Plumbing system in disrepair. At front counter, Water leaking from ceiling AC drain pipe onto floor. Operator stated a work order has been placed to address the issue.

Shell eggs not stored at an ambient air temperature of 45 degrees Fahrenheit or less. Observed raw shell eggs (55F - Cold Holding). Per operator, items have been out of temperature for approximately 45 mins. Item placed in low boy to rapid cool.

Employee with ineffective hair restraint while engaging in food preparation. Observed employee wearing visors with hair exposed in back. Manager provided hair nets to employees.

Food-contact surfaces sanitized with a quaternary ammonium solution not exposed to the sanitizer solution for at least 30 seconds. Do not use equipment/utensils not properly sanitized. After washing item, Employee dipped item in quat Sanitizer for approximately a second. Reviewed proper sanitizing procedures with operator. Items properly sanitized.
Accumulation of dead or trapped insects or other pests, in control devices. Sticky fly tape in storage area with accumulation of flies. Operator discarded.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Front counter - scoops in water 72F. Operator removed.
Time/temperature control for safety food not maintained frozen solid in a freezer. Freezer next to cookline - chicken (39-41F); French fries (41-44F). Per operator items were checked appropriately 3 hours ago. Repair service was called. Operator moved all items to walk in to freeze.
