Observed cut tomatoes (59F - Cold Holding); cooked chicken (57F - Cold Holding); cut lettuce (60F - Cold Holding) in the reach in cooler on the cook line. The operator sated the potentially hazardous foods were pulled from the walk in cooler approximately one hour prior to inspection. The operator moved foods to the walk in cooler and placed foods on ice at the reach in cooler.

Marked exit/path to marked exit blocked. For reporting purposes only. Observed the rear exit path blocked by discarded cardboard. The operator moved the boxes clearing a pathway to the exit.

Stop Sale issued on time/temperature control for safety food due to temperature abuse. strawberry frosty at 9:22 (47F - Cold Holding); strawberry frosty at 10:05am, see stop sale (47F - Cold Holding). Observed manager on duty draining and removing frosty mix from machine, cleaning it, and called maintenance.
Time/temperature control for safety food reheated for hot holding not reaching 165 degrees Fahrenheit for 15 seconds within 2 hours. Observed temp for chili at 71F at 9:40 am. Start time was 8:30 am, with End of Day time at 10:30 am. Manager on Duty removed from bein and put on stove to bring temp up correctly.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed mold like substance in interior of ice machine.
