Ceiling tile missing. Found above employee only door in lobby area.
Ceiling/ceiling tile shows damage or is in disrepair. Ceiling tile above displayed menu board. Ceiling/roof tile actively leaking above displayed menu board manager stated it started leaking this morning, work order is in place. Manager covered station with plastic and water is leaking into container.
Cove molding at floor/wall juncture broken/missing. In Men's bathroom.
Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Manager ran dish machine, found at 0 ppm. Manager stated dish machine is not working, has called to be serviced, and they are using three-compartment sink until fixed. Manager placed do not use sign on dish machine.
Equipment and utensils not properly air-dried - wet nesting. Manager removed wet nesting off pans and plastic containers.
Equipment in poor repair. Door frame missing from reach in freezer next to fryers.
