Equipment in poor repair. Observed self dispensing soda machine in dining room not working. Operator stated that they have a service call into Coke for repairs.

Outer openings not protected with self-closing doors. Observed double doors on East side of building unable to close tightly. Observed right hand exterior door hinge broken at the top right corner allowing door to not fully close, door hitting left hand door when closed.
Exit door locked. For reporting purposes only. Observed single fire exit door leading to drive through lane locked.

Drain cover(s) missing. Observed drain cover in disrepair and missing in mop sink.
Floor soiled/has accumulation of debris. Observed floor in walk in cooler soiled under shelves and on sides of cooler floor.
Light not functioning. Observed light bulb closest to door inside walk in cooler not working. Operator replaced light bulb.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed build up of food debris on walk in cooler door gaskets.
Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed rust on the walk in shelves in walk in cooler.
