Observed employee rubbing head and hair and begin working at drive thru, no handwash. Coached manager on proper techniques for handwash.

Certified food manager unable to answer basic Food Code questions pertaining to safe operation of establishment. Observed manager unable to calibrate thermometer. Inspector calibrated thermometer with manager.
No soap provided at handwash sink. No soap at hand sink at kitchen area. Other hand sink is being used at front counter.

Shell eggs in use or stored with cracks or broken shells. See stop sale. Meat well, observed 1 broken raw shell egg inside the tray with intact raw shell eggs. Operator discarded.
Shell eggs not stored at an ambient air temperature of 45 degrees Fahrenheit or less. Reach in cooler, meat well: raw shell eggs (54F - ambient temperature ). As per operator, placed in the meat well 1 hour before the inspection. Operator placed the the raw shell eggs on ice.
Employee has not received adequate training related to their assigned duties as evidenced by lack of general knowledge in performing their assigned duties. The employees and general manager do not have knowledge of how long the meat patties are cooked on the flat top, the meat patties are by sight. Cooking temperatures are documented on a shift basis. Training is based on a training video 3 stages.
Equipment in poor repair. Observed the meat well reach in cooler ambient temperature 49-52F. As per operator, the breaker keeps tripping off. Reach in cooler, meat well: raw shell eggs (54F - ambient temperature ). As per operator, placed in the meat well 1 hour before the inspection. Operator put in a service call for repair. Operator placed ice inside the meat well and checked the circuit breaker, flipped it back on. checked 15 minutes later, the ambient temperature of the meat well at 39F.
Reach-in cooler interior/shelves have accumulation of soil residues on the bottom shelf of the salad reach in cooler.
