Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Opened raw chicken stored above and in the same box with opened fries and opened french toast sticks in reach in freezer by fryers. Manager rearranged all products to proper storage.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Sliced cheese on cold bar at make line, per manager, cheese placed on cold bar from walk in cooler at 6:00. Temperature taken at approximately 9:30. Manager moved cheese to walk in freezer. Temperature taken again approximately 30 minutes later at 39F.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Vacuum breaker missing on threaded faucet with attached hose in dumpster enclosure.

Operator unable to provide proof of training for six employees hired over 60 days. Operator states due to new year and new system, proof of training for all employees is currently unavailable and issue is being researched.

Equipment and utensils not rinsed between washing and sanitizing. Observed employee wash a container and put directly into sanitizer water in the three compartment sink with no rinse step. Spoke with employee. Employee rewashed, rinsed and sanitized container.
Open dumpster lid. Lid propped open on dumpster. Employee closed lid.
