Observed in reach in cooler half and half (51F - Cold Holding) per operator held at front counter in ice prior to being replaced in reach in cooler per operator held out of temperature less than 2 hours. Operator placed in walk-in cooler to chill.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed potato wedges (118-125F - Hot Holding) per operator held out of temperature 1 hour prior to inspection. Operator voluntarily discarded.
Bulk milk dispenser with dispensing tube longer than one inch protruding from the chilled dispensing head. Observed multiple boxes of single serving items ie fry cartons, single serve cups. Operator elevated

Observed chill (91F - Reheating)@9:00 rechecked 111F@9:30 at this rate chili will not reach its hot holding temperature. Steam table unit temperature was set at 2 Operator increased the temperature.
Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Operator stated everything is being washed rinsed and sanitized in three compartment sink.

Establishment has no approved Alternative Operating Procedure. Observed employee placing ready to eat chicken in small containers with no gloves at back kitchen prep table. Operator advised employee to use gloves.
