Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. Observed employee crack eggs and continued working. Employee washed hands and switched gloves out
Covered waste receptacle not provided in women's bathroom. No covered receptacle in female restroom. Manager stated that a new one was ordered.
Drain cover(s) missing. Observed drain open in the mop sink. Employee put drain cover back over drain
Food stored on floor. Observed boxes of potatoes stored on the floor next to drive through window. Observed employee remove boxes and placed them on cart 6 inches off the floor.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup up on pipes in the walk in freezer. Employee removed ice buildup from pipes.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed food debris in the interior of the microwave. Employee cleaned the interior of the microwave.

Incorrect chemical test kit provided for measuring the concentration of the sanitizer solution used in the three-compartment sink and/or dishmachine. Manager uses test strips for Quaternary Ammonia for three compartment sink. Missing chlorine test strips for dish machine.
