Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. At walk-in freezer; opened bag of raw fish inside 1/3 pan sitting directly on top of fully cooked sausage. Operator moved raw fish.
No paper towels or mechanical hand drying device provided at front counter hand wash sink. Operator provided paper towels.

Dishwasher (Chlorine 0ppm). Operator ran machine twice, each time Dishwasher (Chlorine 0ppm). Advised Operator to set up triple sink to manually sanitize dishes.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. At cart inside walk-in cooler; cheese sauce (50F - Cold Holding); sliced tomatoes (46F - Cold Holding); shredded cheese (47F - Cold Holding); blue cheese crumbles (45F - Cold Holding). Operator stated items not prepared or portioned today, cart with items on it removed from temperature control for approximately 1 hour while receiving delivery, returned to cooler.
Vacuum breaker missing at splitter added to mop sink faucet. Advised Operator to install vacuum breaker on splitter with hose attached.
