Ambient air thermometer in holding unit not accurate within plus or minus 3 degrees Fahrenheit. Ambient air thermometer on reach in cooler on cook line where raw shelled eggs are stored reads -40F when ambient air in cooler is 35F. Manager put new ambient air thermometer in cooler.
Equipment and utensils not properly air-dried - wet nesting. Above 3 compartment sink, pans stacked before properly air drying.
Floor, walls and/or ceiling soiled/has accumulation of debris. Floors under fryers and cook line soiled with grease.
Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup up on floor of walk-in freezer. Manager stated she has been working on it this morning.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On cook line in reach in cooler, sliced American cheese (56F - Cold Holding). Manager stated it was pull from walk-in cooler and placed on line at 7 am. Sliced cheese was in placed in shallow plastic pan inside metal pan in reach in cooler. Manager removed cheese from reach in cooler and put in walk-in cooler to bring temperature down.
Floor, walls and/or ceilings soiled/has accumulation of debris. On cook line behind fryers has build up of grease and food debris. Also, at drive thru area behind soda machine floor has build up of debris.

At drive thru area, employee dropped plastic cup on floor, picked up from floor, throw away and then prepared a drink without washing hands.
Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. At cook line reach in freezer two open bags of raw chicken stored over one open bag of fully cooked sausage.
Handwash sink not accessible for employee use at all times. At drive thru area, tea urn and frosty machine block clear access to handwash sink.

On make line in kitchen, sliced cheese held on time has three different labels. Manager identified that two labels are inaccurate, but one is. Manager removed inaccurate labels.
