Observed three compartment sink next to office with quatanery ammonium sanitizer solution testing at 0ppm. Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee handling phone then immediately went to preparing food without changing gloves or washing hands. Manager coached employee on proper hand washing techniques.

Incorrect chemical test kit provided for measuring the concentration of the sanitizer solution used in the three-compartment sink and/or dishmachine. Operator has chlorine test strips, uses quat sanitizer in triple sink.
Equipment and utensils not properly air-dried - wet nesting. On clean dish shelves above triple sinks, metal pans have wet nesting. Operator moved pans to air dry.

Reach-in cooler interior/shelves have accumulation of soil residues. Under counter reach-in cooler at front line sandwich station, soiled interior.
