Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee handle fryers with gloves on, then with same gloves on make a chicken sandwich without washing hands. Addressed immediately and person washed hands.
Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Missing on chicken and egg sandwiches. Person in charge turned time stamp on.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In back reach in; eggs (59F - Cold Holding) They took the eggs off the top of the counter and put them in the walk in cooler. Reach-in cooler; feta cheese, cheese, shredded cheese, lettuce, tomatoes, 54F; ( - Cold Holding). Items in cooler 2+ hrs. Placed back into walk in cooler. Drive thru; salsa 49; ( - Cold Holding). After 1 hour temps at 50F. Could not determine if cooling correctly.
Employee with long fingernails working with exposed food, single-service items or clean equipment and utensils. Observed person on cook line with long nails poking thru gloves.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Not all staff have signed paperwork. Emailed to operator during inspection.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Mold-like substance observed on left side of ice machine plate.
Bathroom facility not clean. Toiled was soiled, toilet paper observed on ground.
