Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee wearing gloves touched side of apron while on cook line then left work station and handled bags of bread, then returned to cook line wiped hands on side of apron then took food wrapper and prepared an order and grabbed a hamburger bun without removing gloves, washing hands and putting on clean gloves. Discussed with Manager.
Handwash sink not accessible for employee use at all times. Spray bottle in hand wash sink by dishwash area
Cove molding at floor/wall juncture broken/missing. Cove molding separating behind fryers and has accumulation of grease and oil bend the cove molding.
Old labels stuck to food containers after cleaning. Old labels not removed from food containers before cleaning, stored on the clean ware rack.

Observed the following item and cold holding temperature in the reach-in Cooler on cookline: Hamburger (46, 48F - Cold Holding). Employee advised that Hamburger had been in the reach in cooler for 90 minutes and had previous been in the walk-in-cooler. Manager placed all the Hamburger in the walk-in-cooler.
Handwash sink used for purposes other than handwashing. Observed ice in hand wash sink by front server station.
Food stored on floor. Observed box of Ground beef on floor in walk-in-cooler and two boxes of lemonade in walk-in-freezer. Manager head all items placed on shelf.
Unwashed fruits/vegetables stored with ready-to-eat food. Observed unwashed produce (tomato ) stored over ready-to-eat food ( cut leafy greens ) in walk in cooler
