Ceiling/ceiling tile shows damage or is in disrepair. Observed a ceiling tile starting to hang in the corner over a food prep area in the kitchen near the dish machine.
Food stored on floor. Observed a box of oil on the floor in the kitchen operator picked up.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Sanitizer reading of the dishwasher 00ppm. Manager stoped using the dishwasher, set up the three compartment sink and called the technician.
Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. See stop sale. All items on time as a public health control stored on the front make line with no time marks. Manager stated the items were stored on time for approximately 30 minutes. Manager placed time marks on the items.
