Employee has not received adequate training related to their assigned duties as evidenced by the inability to answer basic food safety questions. Employee taking temperature of cooked chicken on grill on cook line stated the temperature of cooked chicken should be 185F, manager educated employee that the temperature should be 165F.

Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee on the main cook line wash hands, begin to put on gloves, drop a glove to the floor, pick up the glove and discard then place on new clean gloves with no hand wash after touching the floor/soiled glove.
Toxic substance/chemical improperly stored. Chemical sanitizer spray bottle stored on prep table over tray paper liners. Operator relocated the sanitizer spray.
Equipment and utensils not properly air-dried - wet nesting. On the push cart in the warewashing area, clean containers stacked while wet.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Bottom door of the two door standup reach in freezer on the main cook line has debris build up on the top gasket. Operator had employee clean the door gaskets.
Utensils in poor condition. Plastic container used to store sliced cheese on the cook line has crack through the top lip of the container. Operator replaced the container.
