Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Carton of milk in flip top reach in cooler with no date marking. Per employee, he arrived at 9am and milk was opened.
Food-contact surface soiled with food debris, mold-like substance or slime. Can opener on back prep table has dried debris on the blade.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Establishment utilizes chlorine as sanitizer in the three compartment sink. No test kit available.
