Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee handled raw beef then changed their gloves and handled clean utensils without washing their hands. Employee removed the equipment to be sanitized, washed their hands and put on new gloves.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Chicken (67F - Cold Holding); Shrimp (68F - Cold Holding); Egg wash (68F - Cold Holding) were the only items stored in a small top load cooler unit on the cook line. Manager stated the items were stored in the cooler for approximately 2 hours. Manager voluntarily discarded the items.
Toxic substance/chemical improperly stored. Spray bottles of stainless steel cleaner stored next to Togo containers in the wait station area. Manager removed the stainless steel cleaner.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.
