Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Backflow prevention device missing at hose connection on side of building, near dumpsters.
Food-contact surface soiled with food debris, mold-like substance or slime. Buildup of old food on can opener blade.

A minimum of one bathroom facility is not available for public use. Public restroom sign covered with "out of order" sign. Manager stated the restroom facilities are not broken. Inspector had the manager remove the "out of order" sign during this inspection.

Cardboard used to line nonfood-contact shelves. Shelf at cookline and shelf in back of kitchen lined with cardboard.
